I don't think anyone "really" want to have sex with their family members. I would argue it's more of a fantasy, akin to having sex with the perfect sisters or brothers or parents. And by perfect I just mean super lewd and very attractive.

Except a bit for a particularly attractive cousin, I have never felt any sexual feelings for a family member. Nor have I ever had any awkward exposures to or by any family members. In fact, I have a very strong aversion to actual real world incest. But I find incest erotica super hot! You can blame the Japanese for this one (Hentai and AV's). I've consumed so much of it, and so much of it has the theme, that it's basically a minimum-level fetish at this point. So much so that, in my case, it barely registers as anything at all. It certainly doesn't translate at all to real life for me (despite the fact that I do have some swimsuit-model level cousins). In some games and porn, incest themes make the creator produce a better product. They put more time into the characters or the story and create more options.

When I got used to incest themes it became a cheatcode in my brain. An incest relationship automatically makes me imagine more romantic or more "you are getting away with something you shouldn't" things between the characters. Both options are hot.

Personally, it's not the incest in and of itself that makes the game so much better than if it didn't have it, it's about the idea that i can basically just get with whoever I want to.

It's a porn game, what's the point of creating this hot character and putting the character in the game if it's then impossible to be with them? Honestly just a waste of time and ressources.
The inclusion of incest removes the frustration and waste of having a great-looking character who's there for... decoration or some shit like that, but certainly not any meaningful reason.

Incest is in some ways the perfect kind of taboo. It's just "out there" enough to have the lure of the forbidden. But it generally is not illegal, and there is not the same level of instinctual revulsion as some other taboos.

Good question, maybe my marriage isn't want it was in the beginning, and a sister as extra love is a nice idea. Or wanting to have had a sister could also be a trigger. I probably wouldn't be interested in sister-incest if I had a sister. At least part of it is the taboo nature of it. And for myself , it also allows a deeper layer of fantasy as I was a single child so the whole sibling incest situation was immpossible anyway.
