If the game doesn't support remapping I'll use auto hotkey

If 1 handed truly isn't an option I don't play it

It varies on how I am feeling that day, but almost never will I play a porn game that requires two hands. The only exception I can think off of the top of my head is Last Hope (The unreal engine space game), since it's in 3D and is free roam, it needs two hands to play, but the sex scenes require 0 hands. Needing two hands kills the fun for me for obvious reasons.
