I'd really have to say its the first three choices by far. Leaning towards graphics though.
A good story is very nice, but if the characters are just a pain to deal with then it really draws for the tastefulness.
However, if the lewd scenes made look nice enough, or if simply, the angles are spicy enough; it will keep me coming back again and again.
This is part of the reason why I love Third Crisis so much. the art is great, and the angles the sex scenes capture are lovely
In most cases for me it's the characters, if the story is good but the characters are poorly written it's hard to enjoy it. A poor story with great characters can work just fine.

I am QoL voter - nothing kills the game as much as the need to alt-tab for walkthrough/stumbling into closed doors. Heck, i no longer play any RPGM games because of how unwieldy UI feels.
For a game to reach the top it needs to be at least decently good in most if not all categories. But I think the most important aspect is that it needs to be unique in some way.
