There are a few things that annoys me.

Posing with hands behind the head or back is very unnatural and devs should do away with it.

When taken a girls virginity there is no blood.

Not allowing pregnancy, especially in incest games.

Forcing anal sex on the player, all sex should be up to the player.

Stupid Protag, too much filler (actual story is fine), I just don't play grindy games (doing same shit over and over for RP); if you want to pace it out then do it with real content.

Any grinding whatsoever. Worse when they add shit I hate that wasn't in to start - fantasy, sandbox, futa, ntr, male domination, etc.
Also, when it gets abandoned.

Mine is a technical thing, but when game have loud ass audio on start. We’re talking like ear splitting. It’s only been really bad a couple times, but those audio settings gotta be tuned down asap.

Not much really bothers me, because I can usually understand why the devs did what they did, but if I had to choose something, it would probably be a bad UI or bad HUD.
