Bad UI with also either no customization or no built-in options for renpy games (the ones from the 'A' shortcut key). Some devs are really messed up and shouldn't be allowed to configure their own UI for their games.

Bad lighting that it make everything needlessly dark, It should be a easy fix but most people never do.

Agreed, some devs really do have issues with how they handle their lighting. Many games get too dark (visually, not script) and ruin scenes (I remember a couple scenes in But I'm The Bad Guy got ruined because of it for instance).
Honestly, blackmail. It just never feels right. It should either be actually properly dark, wallowing in the destruction of some poor soul who doesn't deserve it or an excuse for someone to do risky shit they wanted to do anyway, but it always ends up being this weird thing in the middle that just doesn't work.
Time sensitive games. Where you have to do something on a specific date and there is no skip to date button.
Most points mechanics where its just choose the option that gives +1 love stat, as if I'm not just gonna use a mod/walkthrough. Just make it a kinetic novel and write well enough people get invested instead.
When it gives you 2 choices but neither one changes the dialog at all. Like what's the purpose on the choices, just remove them entirely.
Trust me, you wouldn't say this if you played games like A Mother's Love or Intertwined. It gets so tricky to get the content you want.
Personally it really annoys me a lot when in games on the theme of incest the game starts directly with sex scenes between the family. the first reason is that it's sorely lacking in realism so I'm no longer involved in the game and secondly, frankly, it's much more exciting when you are tease before!
Oh yeah I've played plenty of games that have good points systems. But I've also played plenty that are simply pick the option with the points, have points to see sex, with no further nuance just get point or don't. Those are the problem to me, though tbf its often new devs that end up fixing the whole system later.
