Depends on the type of game, really. But ive had a whole rant in another thread about the unrealistic and disproportional boobs in both these games and things like comics/hentai. Drives me nuts, and I hate it. Make me believe the person is a person and not a parody of a person. The only exception to that is if it actually IS a parody and the disproportion makes sense for that parody. Otherwise, gimme my normal range of sizes and shapes of bodies.

VNs with elaborate intros where the dev has one or two of the characters address the player and introduce the VN and explain how to play it (for those of us who are unfamiliar with, um, clicking) and talk directly to the dev and make a bunch of lame jokes. It's never as entertaining as the dev thinks it is, never necessary for understanding what to do (it's a VN... click) and mostly just amounts to a tedious waste of time before I can actually start playing the game.
Sandbox games where it becomes apparent that the sandbox element is not intended to let the player explore the world or make decisions about how to progress, but to pad the playtime out with needless grinding- the "you need to do X so you can collect Y so you can see a scene with girl Z but you can only do X at night on Friday and Y is only available on Friday afternoons and Z will only talk to you on Friday mornings so there's no possible way to finish this barebones quest in less than three in-game weeks" type games.
Games where the main character continually acts like an idiot or a child for no good reason, and the game clearly expects the player to think his behavior is not idiotic or childish.
Kind of a minor one, and maybe controversial, but games where the writer doesn't bother to... I don't know, justify? address? the fetish at play. By which I mean, say, an incest game where your sister falls in love with you, but nobody ever acts like that's anything more than mildly unusual, and the actual falling in love consists of her seeing your penis once and immediately becoming obsessed with it. Or a harem game where a dozen girls fall in love with you and none of them blink twice at the fact that you're screwing all of them. I like both those genres but sometimes it feels like devs just skip over all the interesting parts of a fetish to get to the end.
On a petty note- the fact that every VN character in the freaking multiverse apparently shops at the same exact clothing store that's 50% changing rooms with like three shelves in the front.

"default" music - No, those stock music tracks you found do NOT enhance the gameplay in any way.
Relying on sound(s) for the game to make sense - some of us play with the sound off.
Non-skippable animations/transitions - Maybe I don't wanna spend 4 hours watching your janky animation of a character walking towards me!

To tell the truth, lately a lot of things have irritated me, either I'm more demanding or it's a midlife crisis.
Personality changes have MC, it looks like the character hit his head and I'm stupid, only stupid options appear and they will all give the same result so why by choice.
Game where some choices led to a bad ending, but choices don't make sense, I wanted choices to lead the MC to a bad ending; Thinking that LI looks beautiful in the red dress, having comforted LI when she finds out that her boyfriend cheated on her, being nice to her family, the developer after a lot of criticism, and many questions why this would lead to LI killing herself, he leaves the options which leads to li killing himself.
Another is the avoidable lazy NTR solution, if you have good statics with LI she should refuse unless you as MC encourage it.
Example LI and MC are in the place for some reason MC walks away when she sees an NPC saying the classic phrase "Hey girl, grab my dick."
The options for the MC's attitude: Intervene or Or not intervene; This ruins the romance completely, like all the work to get into Li's panties becomes just frustration and all options that show that you don't want an NTR route are thrown away.
If the MC is static Good with Li, I would prefer if she automatically refused. There could even be the option for the MC to encourage NTR.
Or at least at that time you controlled the LI and the player could create the school for her. These small changes really change the feel of the game for me.

Agree grind is a right pain. Sometimes there is some great content hidden behind it so I just use the dev console to get around it if needed.

When there are choices that don't impact the story in a meaningful way also there have been times were i make a decision not to fuck someone for example but scenes in the game continue as if i fucked the character anyway

1. Not putting Version of the game somewhere.
2. Exaggerated body parts. Especially freakishly large boobs and penis.
3. Money grind. Grinding in general.
4. Not being able to edit names, age, and relationship of characters.
5. Bad endings that make you start over or back to a pervious save.
