Steering away from the more obvious stuff, I'd mention the original Personal Assistant. Slowly eroding away the titular characters free will by increasingly subjecting her to psychological manipulation is kinda chilling.

Probably Holy Knight Ricca. That may not be super fucked up by most peoples' standards, but I'm a pretty tame person who likes pretty tame things. Holy Knight Ricca is really good, but it's also very rapey and there's a few scenes involving zombies.

I'm truly surprised I haven't seen anyone mention Friends in Need yet. You can take one of the most wholesome paths in an adult game I've ever seen, but also the dark path is so incredibly dark, I legitimately haven't gotten all the way through it. The post-nut clarity kicks in before the nut even happens. Its that bad. The choices you are capable of making as the MC are very disturbing and I kinda maybe worry for the mind of the dev a little lol. Nicki has been through enough already
(make sure you play with the patch, otherwise most of the content doesn't show)

Deaufosse should probably get some professional help tho... after he finishes Malignant of course, wouldn't want that to go unfinished tho now would we?

I'm a big softy and almost always do the Good Guy route in Friends in Need, so I'm not sure how bad the Dark Path can get. I do love that game, though, excellent writing.

Depends what you're considering to be fucked up:

Ravenous and Personal Assistant: Blackheart Edition both had some very strange parts to them. Derketo Lewd Housemates actually did too for that matter.

If you just mean games where you do bad things there are plenty of Japanese VNs revolving around abduction.

Malignant. i found not knowing for sure if it was all real or delusion, if the MC was under control of the brain cancer or just lost all inhibitions which are two different things. with all the fucked up shit in that vn, i find the uncertainty of what happened and what caused it to make it exceptionally disturbing. at least with other dark games its like, okay this MC has zero feelings, or this one is a perv, and this one feels justified. Nope, not with Malignant. One moment you think MC is letting his perv fly, the other he's got no control of his own actions, the next moment thinking wait did that actually happen or was it imagination. bah, i almost want the game to be updated and finished just so i can find out cause morbit curiosity is a good friend of mine. On the other hand, the MC's behavior goes against the little moral code I have I rather not see it. unlike red room, the suspension of disbelief plus the hackneyed gore and how the MC and friends who watched people get butchered live online felt like victims when they were "forced" to participate in person but never really saying, oh shit we were wrong to have ever supported this shit. nope just talk about being victimized and wanting revenge. which i know there are real people in the world that are that self centered, but i hate them, so dont want to play a game where im supposed to want them to win.lol
