Free Cities likely has some of the most deranged content I can imagine, I'm just numb to it now. There's another slave training game too that I can't remember rn.

But years ago, I stumbled upon an online text based adventure game that was something neverland. I've never seen anyone discuss it, but it was the first game I played which incorporated Smol characters into the world. While I don't remember there being anything particular disturbing in it, I found it so because of the Smol content.

Personal Assistant is right up there... but if weirder equals more fucked up I would say Mother Village... the MC is a dog with lowkey mind control, the translation is awful, and there is NTR from a sentient gorilla. Not very dark themes, but it is fucked up both as a game and plotwise.
