Sisterly Lust might hold the record for most babies had by the protag in the epilogue, and the dev's second game, Tales of the Unending Void, seems to be heading that way, as well. Parental Love is another good one. Oh, Now and Then fits into that category, as well. Away From Home has a couple pregnancies so far. My Dorm has an absolute ton. Midlife Crisis had one pregnancy and seemed like it was heading for at least a couple more. Sorcerer had a few, as well.

I can't remember the name of the game, it's something like Incestuous Summer or something. It's a 2d hand-drawn visual novel about a boy who has sex with his 2 sisters and his mom (I guess that isn't really specific)
