Problem with incest is mostly that there is increased risk of recessive genetic diseases if your family has some bad alleles. But it's not like incest equal automatically crazy retarded monster. Nobility got those problems because of really close family marriage during centuries, they accumulate the bad alleles.

In parts of Appalachia this has occurred. A really interesting case is the Fugate family, (look em up) they were relatively isolated from the rest of society for nearly 200 years. Their inbreeding led to several medical problems and recessive genes being passed down through each child, such as one that gave some of the offspring blue skin. The blue skin gene is a recessive gene that only appears when both parents are carriers of the gene. The inbreeding caused the recessive gene to appear in both parents. So "Avatar" people really exist.

On the bright side if something did crop up - you can probably take care of it with some dedicated out-breeding or as I prefer to call it -- Fucking randos

Also today it's possible to do some genetic screening, many of such bad alleles can be detect and preventive measures can be take (contraceptives, using a sperm donor for procreation...) : it's a bad idea though to say you want a genetic screening because you want to fuck your sister.
