Management games with a lot of sandbox and optional quests and goals, but a big overall story to be followed are my favorite because I like to create my own house rules and character details. I like the surprise to come from unexpected societal changes to respond to, rather than have someone's ideal NPC pr MC pushed on me.
I also like working up from having little to having everything but the game making that more easy than grindy. So I would make a Dungeons and Dragons kind of experience where the MC starts off with little power but good abilities and enters some new place and adapts to it, getting more power and fame and control along the way. And high magic or fantasy science would enable the sexual content I want, without requiring sacrificing it to be able to do normal gameplay things. Tradeoffs between achieving nonsexual game objectives or sexual desires is annoying and pointless to me because I don't like realism. I play games for the fun not the simulation.
In the end the MC would rule a large area and have whatever relationships they want and could dictate the relationships between other people (or choose to not do that).
Integrating AI to generate unique content in response to random seed values or the context of the playthrough would be nice, and having the AI generate nice images of the characters based on the descriptions you set for them would be good too.
