That might work if it's just a difference in genders (or it might result in a gay story that people feel is lazily "find/replace all" patched from the straight version). It would be a lot harder if we're talking something like straight vs. feminization, where the entire structure of the story is going to be different.
And of course this is all extra steps that the dev has to take relative to the fetish they're really interested in writing about, which is my point- the thing they're into is a passion, the thing they don't like but are including for money/other people is extra work that isn't personally appealing to them. And it tends to show.
I understand perfectly what you mean and it's probably the case for many but it's not necessarily a reality for everyone.
Some people just want to do well when developing a game and want to bring something rich to everyone who support them and when obstacles arise will not hesitate to ask for help from people better placed to achieve their goal
This type of effort has already been made by classic game studios to implement features or respond to the demand of their community.
Personally I'm a bit of a perfectionist on the edges and if as a dev (which I'm not) I wrote a game I would probably move towards a sandbox as I love freedom and I would certainly want to achieve the best of myself on all the aspects that I would address and from the moment I had a doubt about the quality of my work I would seek the help I need
and yes I would probably put more passion into the things that attract me, I don't deny it either
But I can perfectly imagine writing a gay or lesbian narrative arc according to the sex of my MC even without being personally interested in it, just because for me it would be the logical continuation of this freedom that I would like to offer to my community
On the other hand, I would not write or work on tags that repel me, those that I qualify as redflags.

If I were a plumber I would do my job to the best of my ability and I'm not a fan of pipes but just a fan of a job well done
On your first comment you suggest that you would ignore these Transformation/Sissification tags just because of the quality of the work provided by the dev hence our discussion
