Like most people, gotta go with Desert Stalker, though I'll check out some of the other stuff mentioned here. There was this one zombie apocalypse game with a red headed sister that I thought was very well done a few years back, but it was abandoned I believe. Wish I could remember the name.

In terms of non-adult games I haven't played anything that tops New Vegas, another kinda cliche answer I'm sure.

Wasteland, from 1986 or 1987. Far superior to the inferior Fallout. Better setting (though in in the nuclear war happened in 1998) without the weird, pseudo 1950s stuff. Just the original, not the sequel made more than 20 years later that they made more like the fallout games. Those 1980s text combat systems were the best RPGs.

For me Atom RPG is up there its basically OG Fallout top down isometric view and everything set in a Post Apocalyptic Soviet Russia this game is a true RPG choices actually matter both good bad and neutral you can join a death cult and become the enemy of every other faction and settlement if you want to you can also contract an STD by not using a condom when having sex you can even take part in a freak show porno the game is wild
