Depends. I usually turn music down, especially if it's some kind of pop crap that really doesn't add to the experience, but I will leave "sounds" on, usually to get ambient and event based stuff. If it turns out to be annoying as well (e.g., Japanese porn actress sounds), then I'll probably mute everything.

Mostly I keep the sound low just. loud enough to hear if there are any sex sounds that may enhance the experience.

I play Japanese visual novels with the sound on, because they have good voice acting by professional seiyu. Also, music is usually original and adds to the atmosphere.
In non-Japanese VN, it's usually not good, so I turn the volume down and sometimes even turn it off.

Sound very low, music even lower for the first moments of the game, then off unless really good/part of the ambience.
