It's the period of care, help, comfort, and intimacy (assuming those are things the sub wants) after a BDSM scene. The dom basically dotes on/takes care of the sub, helping them clean up/recover, showing them how much they mean to the dom, and generally building them back up while the emotional or physical effects of the scene fade. It will generally differ from pairing to pairing, but fundamentally, it is the dom demonstrating their gratitude to the sub for giving up so much of their own autonomy and control during the scene itself, and thanking the sub for trusting that the dom would know the sub's limits and keep the sub's actual welfare in mind at all times.

The actual details could be a lot of things, but generally it's the dom doing everything they can to help the sub regain comfort and return to "normalcy" post-scene. It may also involve emotional/mental adjustment, too; for example, if the scene had a lot of humiliation/denigration roleplay for the sub, aftercare might involve loving praise and positive affirmations to offset the feelings engendered by the scene.
