It's mostly the language of it that bothers me. I grew up in a big family with four 'step-siblings' and at not one point in our entire lives did we refer to each other as 'step-sis' or 'step-bro.' We were just brothers and sisters. Characters explicitly (and often constantly) applying that prefix make the dialogue feel unnatural to me.

If the characters are older and didn't grow up together it feels less forced but only if the writing is sufficiently subtle about it.

I understand that most developers do this to allow their game to be sold on steam but I don't know why the dev themself doesn't make a patch or allow for you to name the relationship in those situations. It feels very odd and for a niche and taboo concept like porn games I'm surprised that it exists.
