The beginning is very different, a brand-new, steamy scene. Everything else will be exactly the same, but with new images, and version 0.2 will include what was written but not yet produced, and will conclude the prologue.

I mean the game gives the ability to change the relationships, and openly refers to them as aunt, and cousin before relationship change so it CLEARLY isnt necessary. I'm just wondering if it will be required for someone else later in the game...?

What do you mean it can't be enabled? It is enabled in your screenshot, the 3 checkboxes aren't aligned for some reason (dpi scaling on your device maybe?) but it's clear the third one is the incest one and it's checked

That is for people who aren't here for the incest and also think that calling everyone landlady and tenant is stupid. In this game it disabled the path for Patricia and makes the relationship confrontational (since she did abandon the MC and his sick father after all.)
