The taboo makes it hotter for sure. Not sure why it often needs to be patch in as an option though. Obviously it's frowned upon in most places, but then again, so is rape and a lot of other deplorable acts that we have dedicated tags for. People can be weird sometimes.

It is not people. It is authorities that criminalize some things that were perfectly OK through pretty much most of the human history and give a green light to other that were always frowned upon.
For example, prior to the modern era by the age of 18 most of the girls in most of the human societies would be married and have kids. but now it's a horrible crime. On the other hand, same sex relationships were frowned upon and sometimes even criminalized, and now the Western governments actively encourage them. Just think about it...

Incest was mostly a a taboo but ruling families in some cultures were OK with that and in some cases it was even a common/preferable practice (think of ancient Persian/Egyptian kings and the Incas).

It's the taboo thing and come to think of it, it's the low hanging fruit of taboos. The game can still be rather vanilla, the forbidden nature of it adds immediately recognizable tension, which can be harder to pull off with two "random" people. I think the teacher student dynamic, for which there are a lot of games for is similar.

echoing what others are saying, cultures will always draw arbitrary lines separating what's acceptable, what's shunned but catered to and what's completely unacceptable. i think we just happen to live in a time and place with lines that put incest in the last bucket
