Violence and the other content mentioned in the OP do get censored in other countries when things are distributed by a local distributor, it's just that unlike ye olden days you don't need a Germany-specific version to sell as a physical disk anymore and most platforms are either in the US or pick a European country that doesn't have much censorship laws. Payment processors don't care about violence and gore because it's in movies already so they have to be willing to help sell it, sexual fetishes just are both not part of mainstream entertainment and trigger the pearl clutchers so platforms and processors are more willing to censor those to avoid blowback.

This is just my conjecture based on how humans operate, but I think there are some sexual fetishes that should be publicly condemned because if they stop carrying stigma, it can hurt the society in general. Sexual interaction between close relatives can result in children with genetic defects, we want to avoid that. We may have some problems a generation or three down the road if one day brother and sister dating, marriage and pregnancy become socially acceptable.

Just checked your example. I assume that was mostly a strange coincidence. According to a quick search it was due to the intro depicting real stuff happening at the time and some war breaking out at the same time. Thus the game getting close to being potential propaganda.
