It doesn't really matter much. The original versions suggested that something was going to happen with the MC and his adoptive mom, and had a scene with him and his adoptive grandmother. That's it, but that's enough to require the tag. However, the game has devolved pretty sharply into a heavy futa-on-male focus instead, and IIRC, the dev has said that's their intention going forward.
