Most of the time difficulty=more grinding but no fail states introduced into the game but they don't tell you. I'd play off of easy more if hard didn't just mean dumb sliders set to 11 but you still have infinite time to grind so it's just more of the blueballing that the earlier games mentioned.
Pregnancy is 99% of the time just do you want to creampie or not and maybe a egg+sperm pic pops up. If you want preg content there are a lot of RPGM games where female MC's get knocked up and have kids but it works better in that anime art style. Western 3-d styled games don't have that as much because sexualized female models already are pushing the limits of looking human and when they suddenly get fat it looks off from a bunch of angles. Think 90's superhero bodies and how inhuman their spines and hips are then make them also pregnant and its like a contorted barbie that got knocked up from an Alien Face-hugger.
BDSM is weird for people who aren't into bdsm so making content for consumers that you don't personally like ends up usually with porno level acting which you probably find boring. I feel similarly about most exhibition focused games with few exceptions.
