Well, this may not be enough explanation for you but the sensations are very different from oral and vaginal. And a girl with good hygiene and diet and most importantly willingness to prepare beforehand can pretty much eliminate most of the issues you are referring to. You can also use a good condom to keep clean and also protect against micro abrasions which is where some danger exists in anal penetration.

Now, all that being said, scat fetishisizes the shit itself. I cannot understand nor have I seen an explanation that can even begin to give me some sort of clarity as to even how it is a fetish. And to be honest, I don't really care to pursue the topic anyway.
