I hate step-cest as well. Gimme the real thing (not irl obviously) or nothing at all. Or at least include an option in your game that allows you to skirt around the issue by letting you, the player, give your family members their appropriate title. I believe Desert Stalker did something like that where, in the very beginning of the game, it asks you who this person is to you and you can type in "daughter, step-daughter, adoptive daughter", or what have you.

Funny thing is, to Patreon, etc step-cest counts the same as real incest, so it's not protecting them there.

Step is a legal issue, the reason sites like Patreon flag it is because it's a pretty hollow disguise for the intent. Whilst it's pretty silly to apply regulations to fictional characters who are just a bunch of lines of code laws tend to be written in a way that are purposely vague to make it easier to use them against people should the need arise. Did you know that in Oklahoma it's illegal for a minor to possess a permanent marker? Well it is. Has it ever been enforced? Yes and no. There was one case where a teacher that REQUIRED them for her class actually called the police when the marker bleed through the paper onto the desk "defacing" school property. The police actually showed up and sent the kid home, no arrests were made, but the teacher was warned that since she required the markers they could arrest her for aiding and abetting juvenile delinquency. The point is catch 22 laws exist, loose language that favor the courts are purposely included in these laws which often allows overreach to infringe on the rights of those governed. Including "Step" is just a way to get around most of them whilst sites like Patreon act as publishers not a platform so they are well within their rights to filter out the transparent attempts to circumvent the law because to any reasonable person it's like putting lipstick on a pig, it's not going to trick anybody into thinking it's not a pig. If Patreon were a platform then individuals could theoretically argue it in court that by forcibly censoring their content they are acting as a publisher instead of a platform, but even valid lawsuits take quite a bit of money so it's hard for individuals to challenge corporations which can bleed individuals dry before any results occur often creating a pyrrhic victory even if they are successful.
The bottom-line is it's a deal with it situation. If the game developer isn't willing to implement something that allows you to remove the "Step" you can try to rely on third party mods of the content, but the developer HAS to follow the law and ToS to make the game in the first place.

The politics of money. They say they don't limit what people buy due to their beliefs, but they've been caught lying. Steam and Itch.io are fighting them right now.
How it's done? Groups complain to payment processors like Mastercard & Visa and since almost everyone routs transactions through their networks, they effectively control the market. So, incest, rape, violence against women/children, etc are banned. Some are fighting against it like Japan and a few others, but who want's to come out and say they are pro incest? Not many. They are now going against all forms of pornography and video games now.

Historically, these things come in waves. It tends to peek then come down. So, it's going to get worse, then people will have enough of it and things will scale back to a more reasonable level. I don't think things like incest, loli or sexual violence will ever come back in the mainstream unless it's done on a serious level (not porn or games). The US, EU and Australia are becoming more and more authoritarian mainly because they were too liberal. Authoritarians love to sensor the stuff they dislike - but are often the ones that get caught doing it the most lol.

They are forced to but they usually, from what I've seen gives you an option to keep the step, roommate, and landlady or change it to whatever you like.
