In RPGM where we are forced to use arrow keys, usually I script something to use WASD as those keys, so the single hand can press all needed buttons.

the secret is very simple - I don't play roaming games
or I play with both hands until I find something worthwhile

Thankfully, when it comes to porn, I'm ambidextrous. Left hand doing it's thing while Right hand uses the Arrow Key / Enter / Numpad-0 control scheme.

I dunno if how many users here have played Persona 4 Golden PC edition. It has a neat feature in that the game can be completely played with a single hand, as you can control the camera, movement and all the menus with all the keys around the WASD keys.
Why isn't this the standard approach for all porn games? Even VNs will splash clickable objects and dialogue choices forcing that take up my other hand unless I move the mouse over to be used with my off-hand.

I am interested in making a game with minor free-roam elements, but I think my principle for design is to make sure that players have a hand free at all times. Thoughts?

All Ren'Py games I play single handed.
Sometimes with other engines I user AutoHotKey to map keyboard keys to the mouse if needed, or in cases like RPG Maker I download de editor just to edit scripts as to make it WASD and Space instead of Arrow Keys and Z (and add more QoL, too).

Some set-up required, but I see why you would do it. Ren'Py games with numbered selection menus immediately get a bonus star in my book.
