Doesn't Summertime Saga by now have pregnancy that's not tied to endgame stuff? And you can impregnate a lot of the ladies last time I played it, but that was a long time ago.

There are a few games heavily featuring incest pregnancies before the endgame.
23 Sisters and Daddy's Little Girls both go over the top with this. Maou-Sama goes even more over the top (its kinda timetravel and MC is everyones dad).
The Guardian has a sequel (The Lost Guardian) where many of the characters are ones the MC sired on his family members from the first game.
There are sexmods for Crusader Kings III, which probably offers the closest to a game-of-thrones experience you can go for. The regular game already lets you play as an incestuous medieval dynasty, you only need the mods to see the sex scenes.
My game has repeated incest pregnancies, although with limited options to interact with the resulting offspring.

Yep, you can impregnate Jenny, the sister.
Although you're gonna need a mod for the incest content, the artwork clearly is intended towards the MC (Anon) and Jenny (the sister) being related, as well as Debbie (the mother)

I hate when the girls are finally knocked up and then the story ends, without pregnancy sex even. That was a huge disappointment in DMD4. Sisterly Lust is really good in that regard, unfortunately a bit outdated graphics nowadays.

There are a few good games that include Pregnancy, Sisterly Lust (as mentioned), and Parental Love.
There is an older game too, I think it was called "Dreaming of Dana" or "Dreams of Dana" or something similar, that had the sister getting pregnant with the actual story behind it.
Not sure if the game was finished/canceled as its been years.

The taboo regarding incest impregnation is the basis of the taboo regarding incest (and with some genetic justification, although only really relevant in face of repeated incestual practices in small communities). Still, nowadays, with the ability to perform genetic screening, such concerns are not on the forefront, and as such the social taboo is much more important... Meaning if you are already breaking one, the other is not all that relevant.

Essentially the MC knocks up his sister, then is gone for /whatever/ reasons.
Years later the sister introduces the MC's twin nieces...
I remember they were abused by their mother (MC and sister) which was the 'porn plot' reason they were messed up mentally...
Anyone know which game I am talking about? Or am I drunk dreaming again?

Kinda sounds like Such Sharp Pain. It's a locked game so maybe that's why you can't find it.

I wanted to like it but man that game makes me feel like I'm in a fever dream. Characters just act weird and have strange scene transitions.

I have seen (not played) Such Sharp Pain, so I cannot comment on how similar they are, but I do know that Such Sharp Pain is somewhat newer (like last year or two).
The game I am thinking of is a few years older than that.
It is going to annoy me now until I find it....
I'll post if I can actually get the name.
