For me I needed to run startup.exe from the mounted iso for it to work, keep in mind that I'm playing the VN on Linux. In Windows I believe that after the installation and patching, executing the startup.exe file on mounted ISO will run the game.

Did you used locale emulator? you need japanese locale for the game to run.

you dont need to do that. the folder inside the iso contains the game, copy it to the desired location and apply the patch. use locale emulator to start the game.
