This is probably my biggest complaint too, although I still like it more than without. The blood does not look realistic or has a look like it is an aftereffect, which it most likely is with the dev doing a clean and blood versions of the scene, and while it stresses the moment, it ruins the overall visuals to me.

Do you mean virgin blood? I leave that on. There are some games that have violence in the non-sexual scenes that have a gore toggle - Desert Stalker has one, I believe - and I usually leave that on unless it starts coming off as too much for me or takes a turn towards the sexualized.
