Caring... is kinda tough. Not a lot of wholesome MCs the OP's category.

Most MC's have 1 of 2 extreme mental monologues:
1-"Ass! Tits! Pussy! Must fuck!" or
2-"They've all signed a double notarized documents of consent... but I STILL hesitate to kiss them on the cheek. Let me mentally rehash this 20+ times first."

Plenty of other MCs have Dom personalities. Also, for some players, the MC's harem concept of "I can fuck all of you but you can't fuck anyone else" bothers them.

So... no examples come to my mind. Check the other suggestions on this thread unless you weren't being specific about caring in which case I might have ideas.

That covers Mother, Sister, Aunt, Cousin, (blood with ipatch), as well as friends.

NOTE = There's a heavy amount of MC cucks other men, usually while they're nearby buy they're unaware (i.e. MC is behind the door while the wife speaks, under the counter, partially hidden in the pool, etc...) so if you don't like that kink, or if you like variety, that's a huge amount of the content in this game.

Also, chapters are short and the release rate is 3+ months between each release. Keep that in mind.

Nobody has mentioned two of the Dad/Daughter games that I started out playing years ago. (aside from the first couple chapters of DMD... the story after chapter 2 just got more and more unbelievable, so I found better stories)

Konk Island (don't know what happened to it, it was pretty finished when I played the v0.7 a long time ago and I only have the 0.7 version on my drive)
Lewd Island (again, I got lost on later updates. The original that ended when both Dad and Daughter left the island (she is also the DMD model which was interesting at the time)

For good writing (so far) one of my top 5 is the Petal Among Thorns/A Moment of Bliss (haven't even looked at the third in the series... maybe when I get snowed in in January it'll be ready)

Ow Lewd Island was the perfect alternative to DMD. sadly, the dev had a tight deadline and ended up rushing due to so many criticisms
