Super-short summary: Dude got into trouble for inappropriate behavior as a doctor in another town. He fled authorities and moved under an alias to a new town, but he obviously can't just change his behavior overnight. So he starts things up again, trying to take advantage wherever he can, even if it's sometimes (i.e., almost always) unwise.
