I have seen (not played) Such Sharp Pain, so I cannot comment on how similar they are, but I do know that Such Sharp Pain is somewhat newer (like last year or two).
The game I am thinking of is a few years older than that.
It is going to annoy me now until I find it....

There is one that does this... I cannot think of the name though.

Essentially the MC knocks up his sister, then is gone for /whatever/ reasons.
Years later the sister introduces the MC's twin nieces...

I remember they were abused by their mother (MC and sister) which was the 'porn plot' reason they were messed up mentally...

Summertime saga is really funny when you take the pregnancies into consideration
You can impregnate, like 15 women, waiting for the birth before impregnating the next, and it'll still be the same summer
