It happens, but only when a western (or otherwise foreign) publisher not only buys the game but also is willing to pay the dev extra to put in the work on an uncensored version AND the dev is willing to do it rather than focus on the next project they've moved on to by that point. For an example, see Summer Memories and its sequel Winter Memories - the Kagura releases are uncensored, though if you buy them on Steam you have to get a (thankfully free) uncensor patch from Kagura's website.

ETA: also very occasionally the censorship will be implemented in a way that modders can patch out, but that's generally only in 3D games - 2D games almost always have the mosaics "baked into" the images rather than using a filter you can turn off with enough know-how like in 3D games.

Also a lot depends on if there are actually genitals under mosaic because devs often put nothing in there knowing they will have to censor it anyway.
