My first "real" nsfw game was very recently, a Renpy VN found on a forum, but my first nsfw experience in a game might be The Witcher 3, when you complete your first romance ahah. I was like... Damn that shit is hot!

It was on newgrounds, found a menu tab that lead to some old games which were 18+. No need to register or anything too.
Don't remember the name, but my first one was a sidescroller with demonic floating tentacles in hell.

I remember playing this game back when it was on 3.5" disks, If I remember correctly, the original had an adult verification quiz that was some random trivia questions that I guess they figured no kids would know the answers to in order to unlock and play.
