Can be a text adventure or something. I'm just curious which devs have spent the most on multiple multiple choices that do end up affecting outcomes in the long-run.
 
I'm quite curious too.
I've not played a bunch of games, but among the ones I have (and I'm able to remember right now🤒🤒🤒):
Bare witness has a finale for soloing each girl, two thresome endings, a foursome and a semi-harem.
Summer Scent has a few endings (bad ones), but it's still work in progress and features 18 different (very different) paths.
Leap of Faith has, once again, a solo ending for six of the girls, and two threesome endings.

I'm quite sure there's more, a lot more out there.

Are you thinking of just choices that changes something in general or choices that make changes to the overarching story?

Being a DIK, is the biggest exception that comes to mind, haven't played it the last few updates. I felt that you get different routes you can go depending on your "personality score" -thingy is done in a good way to make the the choices feel more meaning full.

Element-174 has a very broad range of branches- pretty much each choice, and there's several per in-game day, will at least slightly alter your overall arc.

College Daze and Power of Will, by G28, are also massively branching.

Couldn't tell you which has the most branches total though.
 
Kind of a cheat, but Wife Trainer has multiple branches for each wife.
 
The ones I'm familiar with are Love & Sex Second Base. But that's its weakness - lots of characters I personally couldn't remember, lots of harem options and eventually the game just gets boring. I had to make notes in a regular notebook during gameplay to keep from getting confused. Eventually this variability bored me and I deleted this game.
The game is three updates behind here, the latest version can be found on other sites.

Good Girl Gone Bad. Due to the simple 2D art style and simple interactions (outside of the TONS of sex scenes), it was easy to implement many branching paths and characters which change things drastically

I was going to say that one. Never finished because the ammount of routes I haven't done yet were too big, so I gave up. Hahaha
Dog Days of Summer were still releasing when I played it, but it looked like it had a good amount of different paths.

Yeah, even in it's current state, Dog Days of Summer has some great branches. I was actually going to mention it but didn't because it isn't finished and is now being rebooted. It's like a 3D lesbian only version of Good Girl Gone Bad...which makes it far superior in my opinion

Yeah, I put it on hold because since it's still ongoing I didn't know which choices would affect the future, so I couldn't just have the needed amount of saves to see all scenes.
I didn't know it was being rebooted. This usually isn't a good signal, I hope the dev know what he's doing...

He's going to remove/shorten some branches and change things a bit as it's gotten too complicated. The latest and final progress of the old version is available for download along with the reboot. I suggest you play the old version to see all content and wait a few months or years for the reboot to get enough content
 
Solvalley School doesn't have a lot of local choice except NTR-avoiding alternative paths, but it has 26 completely unrelated endings:
1 huge harem ending, 16 regular endings with different love interests (one in a couple variant and a harem variant), and 9 extra endings.

Pale Carnations only has the opposite kind of branching so far. No matter what you do, things advance more or less in the same way, with only some scenes omitted depending on your choices. (This may change as the game approaches its ending, as it is currently only about half done.) However, there is a complex web of small decisions having lots of consequences further on, sometimes much later, in ways that are not always obvious. Personally I think it's a bit too much. I prefer it if there are only a few major branches which I can all play through.

That was also the first for me to come to mind. I think it's great but you are right in kind of suffers in the late game.
There are also so many options and characters that you can lock some things by not doing something or for example dating some one else, i locked myself out from the "Band Harem" and soon stopped playing after that
 
AlwaysFan has some pretty major decisions. As with the similar (but unfinished) Pale Carnations, things still roughly go in one direction but some people's fates can be totally different depending on your decisions. For example, you can protect your girlfriend from the criminals or pimp her out to situations that she barely survives, or choose some intermediate path.

Stormside is infamous for its large number of minute choices that often get their own scene variants with separate renders. I think there is nothing really major yet except the ability to throw out the entire ghost hunting sub-story entirely by forbidding the relevant love interest to pursue the one thing she came to the US for. The renders for choices make development of Stormside very slow, but since this is precisely what motivates the developer, I see no reason to complain.

Biggest i have played so far was collage daze still amazed to find new paths while playing
 
"Monster Girl Quest" is a nice light novel with a massive amount of monster girls, but absolutely no choice at all. Just follow the story.
So massive it got released in 3 parts:




"Monster Girl Quest: Paradox", which comes after it, is an RPG... and that brings all that massiveness in your team: massive amount of teammates to bring with you, massive amount of traits, weapons, etc. The story itself is still quite linear overall, although you can go here and there and do a bit what you want, so not really a massive branching (but still massive in length), but you can get a completely different feel by changing your way to play whenever you want during the game. This one also comes in 3 parts, the last one being really recent:




But, you need to like monster girls and reverse rape/female domination. But it is so massive that you can also find plenty of derived works on it.
