I'd say ideally they look similar as in real life siblings look similar.
That being said with graphics and rendering I think it's a lot harder to portrait that same siblings similarities as in real life.
I'd take the game being good and siblings not looking super similar than just a copy paste version of siblings I think would ruin it.

Never thought about it, but if one kid is white and one is brown while parents are white it will be a bit strange.

As long as they have some slight similar traits and don't look impossibly different then i don't really care having characters look like twins can be fun but also boring depending on how it's done

I think I would like resemblance between them but I also understand that's pretty limiting for a game.
Chances are I wouldn't play a game if they all look similar but not quite to my tastes but I would play a game with a variety that have a few that hit my taste.

In all seriousness, I think that a family resemblance reinforces the entire point of the fetish: the taboo nature of it. I don't think they should look like clones, but very similar physical or personality traits are always welcome in a game like that.
