its not *super* extreme, given I'm not very comfortable with that kind of thing to begin with, but I'd say friends in need has some pretty visceral rape and abuse options when presented. you aren't really going to see any of them beaten into a bloody mess though if that's your kind of kink. it also lets you (mentally) abuse a young woman who wants to be a man by refusing to put up with that and even forcing her to wear women's clothing. I guess that kind of applies?
