Quality sound fx, feels like its a bit underrated, some cheek slapping can go a long way
 
I get it that this engine is limited for many things, but it make me dumbfounded when people uses Unity or ohter engine to make regular visual novels just like Ren'Py is know for, but without this functions...
I somehow understand it, while Ren'Py can do a lot (it even supports OpenGL 3D engine, but I don't think I ever seen anything remotely 3D in it), the fact that you have to code everything yourself falls short to something like Unity plugins where you don't have to even touch the code at all. Makes me wonder by how much would Ren'Py's usage skyrocket if it had a full fledged interactive editor.
The bigger atrocity that makes me question author's sanity is RPGM. From what I've seen that engine is a mess and was already old a decade ago and yet, somehow, it's still one of the most used out there. Like what the heck?

Ren'Py doesn't have a visual editor, but the background images and dialogue text are very easy to do, and some other engines VN do just that, not any more complex stuff.
I've played some RPGM that while is still worse than regular VNs and don't make use of any RPG mechanic, at least was a newer versions, could be maximized, controled with just the mouse etc., but most are older versions that are inded awful to play...
 
Ability to realtime adjust the models in DAZ VNs.
I.e. an age slider that can instantly increase or decrease ages of each model. Apply or remove pubic hair. Alter the hairstyle or remove piercings, tats, etc.

I know it's not how either software works, but damn some of the games I've adored would be utterly perfect with just a few minor tweaks like that.
I'm actually surprised that nobody tried to do something like this in Unity. Basically replicate Ren'Py functionality with realtime animations and adjustable models, and maybe turn it into a plugin, so more devs can use it.

Quite a few - 86 the teasing and any ntr, futa, blackmail, etc.
 
Karryn's Prison with a built in pregmod / filling out the medical center / more corruption options. Know it would be a big art thing but better than the current DLC's coming out imo, Karryn was already stacked.
 
DMD is the first game I played... but the graphics are dated now. I wish it were completely remade and improved, especially with longer, smoother animations and more realistic character details.
 
A lot of free roam games could benefit greatly from being switched to a simple VN. Extra dozen clicks of navigation between scenes rarely improves the game.
Especially when they can't be skipped and are over the top in difficulty like in BaDIK.
