In version 0.50 I was able to remain loyal to my girlfriend, although I seem to have lost a significant amount of content, but here I prioritize moral choices. Curiously enough, there are no outright negative characters in the game (or rather VN). I also feel sorry for the mistress - she is quietly drinking, her fiancé also has a lot of problems, it is very difficult to make a humane choice.
Whether to be NTR or not depends solely on your choice. The characters are well written and the choice will not be easy.
