I think something like sometimes happens in manga sites, with all nsfw content blocked, and you can unlock it and personalize.

For games, i would say, have a cover that is safe, and tags clearly denoted, so even if you enter to see more, reading the tags can prevent you from seeing something you are not comfortable with.
And in search, have a tag searcher where you can denote which tags you want to look, and which you don't want to see anything of.

Another is something similar with "N hentai" website, that once you log in, you can blacklist whichever tag you don't want to see in the general searches, or at least to appear with some warning that has a tag you blacklisted.

Literally Everything... If you are talking about actual Porn then tag filters dont help as most of them add every tag possible to every video so that their vid is more likely to be see.. finding anything specific is kinda tough.

Honestly? I don't think anything should be auto blocked. BUT every site should have a blacklist option and when first signing up, it the first page after signing up should redirect you to where you can edit it, along with a description of how it works and a list of all tags on the site so you can scroll through and easily find what you need to block
