Exaggerated? Absolutely not. Even extreme in the natural range is mostly a turn-off, and so is skewing the range of occurring sizes too much.

It's odd most (including me) prefer realistic proportions yet games with unrealistic ones work so well. Maybe those who appreciate it are to busy with their hands to reply here?

9 out of 10 times it ruins the game. The 1 time it's good is when it's used as humor, but most people suck at making jokes so it's also bad

For girls, I prefer petite and flat, so definitely no exaggerated size increase. I like chibi in non-sexual 2D, otherwise it weirds me out.
For males, somewhat exaggerated penis sizes are fine, as long as it's within reason.

I personally don't like it. but if that is something someones into that's their choice and i won't try to impeded on their enjoyment or fun.
As for why i don't like it, it just feels wrong I've always been really uncomfortable in regards to body modification (taken to the exaggerated/extreme) as a whole.

I think it only works when the whole game accepts the absurdity of it. "Bimbo" games where everyone is insanely exaggerated can work as a style, but if it's the MC with a gigantic dick it's super out of place.

Crazy proportions are bad, but worse is just terrible art/style in general.
