No Addons active

Looks to me like you're using an old theme. Would suggest changing theme, there should be an option at the bottom of the page. Should. Find 'OASIS V2'.

Pretty sure there is another way which is through your account settings if this doesn't work.

No i have not had this issue but this does look very strange
