The next update's gonna take a bit longer.
I was out of the country for a couple of weeks with work just after the last update, so I was already behind before I'd even started.
There's also a lot going on in v.04, with different locations, story progression, and as @TonyMurray pointed out above, pathing complexity, variant renders, etc..
Just know - I'm working hard on it.

Was trying to figure out how old Cara was supposed to be. Her face looks mid teens, and she is several inches shorter than her mother (kids these days are very rarely significantly shorter than their parent of the same gender once full grown, so there is little chance this girl would be that short at 18+), so I'm thinking 15, although with girls she should already be close to her adult height at 15 (my friend was her present 5'11" at 14).
Then she's hanging around in a bar and drinking beer (in the US that requires you to be 21) - although they do say she has a fake id, but unless the barman is blind (or just doesn't give a shit) there is little chance he'd buy that for long, and getting closed down by the cops looking to boost arrest numbers is not really a goal for most people.

It's a small thing I guess, but kinda messes with the immersion in the story.

I think the MC is the same model as Boxed In, and seems like a real mutt to me. Prefer not to see his mug at all, but at least make him such that we can believe girls get hot on sight.

Otherwise off to an interesting start.

Sue meets the Mc at that same bar, so it's possible that the bar is tied to her which could explain why the bartender doesn't care about the girls actual age as long as they buy drinks but also isn't worried about the cops. Just an idea.

As for the look of the girls like Cara, I'd agree, she looks mid teens to me, but I think even though the dev isn't charging so doesn't worry about making the incest real it is still something else to make the girl officially less than 18. That could get it banned from other sites that the dev still wants to the game to be on. So best just to not bring it up and let players decide. Though I think it would be nice to have some official ages, and there were a few girls who aren't yet 18, just so some age play could come into the dialog.

Finally, about the Mc's look: for the older chicks, they likely care less about that, and he isn't ugly even though he might not be attractive to everyone's tastes. So nothing odd there, especially for the ones who knew him before and those who get to know him via the story. As for the younger girls, so far pretty much all of them have daddy issues of one kind or another. So, girls like that will often be attracted to older men even if those men aren't as physically attractive as a younger man might be. So, he can be a little more world worn and it can still work for pretty much every character we've seen in the game so far, not to mention that a couple of the girls seen to have an almost biologically ingrained incest attraction from the Mc's side of the family. So his daughters and niece, and eventually his sister probably, all have that going for them and the other girls are either Cara's friends or the Mc has saved, or will save, their lives and stuff and again... daddy issues.

Includes past characters from the previous title, thus indirect NTR in an environment that in my opinion does not justify it as it doesn't align with the values of the characters prior.

The story is all over the place with no grounding or foundation. The characters are caricatures more often than not and the whole thing reeks of poorly assembled slop.

If all you're looking for is fap material than you'll probably be fine.

If, however, you enjoy a proper story/narrative - you will find this.. disappointing.

Yeah, that kinda ruins Boxed In story in my opinion. He could have just set them in the same world without having the Boxed In character immediately get horny and fucking with Secret and Promises MC.
Once you introduce a story like that, you're sending a signal. People who love NTR will see it, come in, and start demanding more.
