It's actually quite decently sized. Most updates are over 1k renders, multiply that by the 11ish updates and the game is much bigger than you think.

Don't know what you mean by barbie dolls, but the animations are pretty good. It has a really intriguing story. I will say if you are here just for fap material, you will be severely disappointed. There is a decent amount of H-Scenes, but it is first and foremost an engaging story.

Adding to what Stelevein said: Cosy Cafe is closer to a Harlequin romance novel targeting men instead of women, than it is to porn. It's all about creating a strong emotional bond to each love interest. Also, it's (essentially) kinetic. You can't even omit any of the main love interests (not sure about secondary ones) as things would get too convoluted for the developer if you could, and they would never be able to keep up the relatively fast development speed.
The MC is slowly accumulating a diverse harem of beautiful women. He loves each of them deeply and each of them loves him deeply and relies on him as a provider, for protection, and to solve specific problems for her. Starting out as just a last year student who happened to inherit a café in a different city, circumstances put the MC on a path to become a powerful local politician. Basically something like the human version of a gorilla patriarch. A gentle giant protecting his large family of adult females (and maybe at some point lots of children, though so far no pregnancies). Protecting them against the patriarchs and unruly young males of other families, and against the rest of nature.
Sex is part of all this, and there are sex scenes. More than would be necessary to tell the story, and especially the spanking stuff is a bit incidental to the plot. But all the sex scenes are very well prepared to be meaningful in a way that elevates this AVN above the usual porn.
Cosy Cafe is not unique in this. There are a few other AVNs that have more or less the same quality, such as Now & Then (which differs primarily in that everything takes place in a zombie apocalypse and all love interests are optional) and arguably also Eternum (which is less romantic). But these AVNs, that are erotic without being pornographic, are rare gems of mature male wish fulfillment. If the story hits you in the way it should, you will stop caring about the details of what the girls look like. It won't matter because you will care about them as persons. (Of course there is no guarantee it works on every man. Some may consider the story shallow or fake. I pity them, as they are missing a beautiful experience.)

I personally like all the girls in Cosy Cafe. It is one of my favorite games. If you are looking for more diversity in the girls, maybe try The Fiery Scion, while not as pretty as CC, there are a lot more girls.

As far as the next update? Late April - Mid May.
