Monster Girl Quest Paradox goes pretty in depth with rpg gameplay, that shit is better than pokemon. i'd also add the Sequel series and Black Souls, though black souls feels more like a game that has porn rather than a porn game proper.

I have a few games to add, some well known, others less so:

Syvaron's Portals of Phereon. Probably the best hex-based tactics game I've played.​
Mado's EroDungeons. Darkest dungeons, but lewd. The mechanics are familiar, but it's different enough to be interesting. The dev makes great little strategy & tactics games. I'd also highly recommend EroHunters and EroWitches.​
XXXivilization. Strategic conquest with sandbox and RPG elements later in the game. Chapter 1 has better gameplay IMO than Chapter 2. Also features a rather... memorable sex-combat system. Worth checking out.​
Fenoxo's Corruption of the Champions (The OG) & Trials in Tainted Space. Solid text-based open-world RPGs with fun combat systems. Trials In Tained Space arguably has enough subsystems to qualify as an immersive sim. As for the ero content, it's mainly monster girls and futa.​
Strive4Power's Strive series. Combination slave management sims and RPGs with fun combat systems and slave training.​
Free Cities. While not especially complex, arcology management, slave trading, and future societies manipulation makes for an enaging little economic sim.​
Westane's The Company. Probably the best lab management game other than Lab Rats 2. Lots of corruption content and many branching paths for characters.​
Wape's Incubus City. A dark sandbox game focused on the player impregnating unsuspecting women. The gameplay seems simple at first, but has more depth than one might think, mainly centered on managing karma and extending your run through secret methods.​
Kittery's Paraphore. Surprisingly complex and engaging metafiction, if ya can stomach the twisted content. Not for the faint of heart. It's great!​
Indivi's Lust Doll+. A solid free-roaming sandbox adventure RPG. Broadly similar to Lilith's Throne, but with more of a story.​
RahiMew's Broken Dreams Correctional Center (BDCC). An open-world sandbox game set in a space prison. Also similar to Lilith's Throne, but with more content and a nice streak of brutality.​
Perveloper's Captivity. It's a pretty solid little 2D shooter, in the style of old flash games.​
Vrlenir's Degrees of Lewdity. The best orphan sim on the market. Be a Dickens character! Get fucked up by everyone you see! Also contains surprisingly fun market mechanics and tons of depth to the world.​
Godspeak's The Silkfang Respite. Kinda like pokemon, but you can spank the monsters you catch.​
Anduo Games' Third Crisis: Neon Nights. I've only played the public demo, but it is shaping up to be a high-quality immersive sim. Cyberpunk flavored.​
 
if you add zombie retreat, you can add the following too, they are similar RPGM + walking around solving quests:
  • treasure of nadia
  • the genesis order
  • symphony of the serpent
  • milfairy tales
 
Mystwood Manor and Quickie: A Love Hotel Story are both small-world sandbox VNs with a hotel management game as their core. I replay them more for the management games than the scenes these days.
Yeah, the task assignment system in Mystwood Manor was very well done. I'd definitely agree that managing the hotel was the most interesting part.

Well, that and the maid outfits...

If you are into roguelike dungeon style first-person horror game, I would give Sin Spire a try. The porn aspect is used as the game core, though the grinding does take away a bit of fun later in the game if you don't plan to blitz through the run.
Enemies do get repetitive in the later part of the game.

Tales of Androgyny by Majalis [ongoing]
If the art and the flavor is for you, this game is for you. Even if not, it may be for you. Consistently some of the silliest word salads being served up here.

`Chainmail Bikini Squadron` by McRibbed_For_Her_Pleasure [ongoing]
Early development days but fun gameplay is a high priority

Tales of Androgyny is an excellent recommendation. It's super well made and the combat is meaty and fun. There's also a good amount of content for dominant PCs, and the instant loss/sex-on-loss events can be strategically avoided once ya figure out what to do.

I like Courses of Temptation too (it's got solid lifesim mechanics and there's some great exhibitionism content, especially at the sports bar/restaurant off campus). Definitely still at the early alpha stage, but it's steadily getting more content and has a really solid modding community.

Haven't played the others yet. I'll have to check em out. :) Nakara, Castle of Temptation, and the dungeon keeper ones look right up my alley.

Haven't heard of "Strive to Survive". Is that different from Strive 4 Power?
 
I was really surprised by the game "Parasite Inside". It’s exactly the sort of game I’ve always wanted. Sci-fi spaceships, aliens and great sex scenes.
 
For a game to have a gameplay status for you does it have to be sandbox? It is a must for me, it immediately gives me the sense of the game actually being a game.

Yeah, I mostly agree! The sandbox stuff is kind of what appeals to me about non-traditional computer games and interactive fiction in general, that sense of having a world to explore outside the constraints of reality or a preset narrative.

Bonus points if the game has stuff to do that isn't directly related to the nsfw content. Like, it feels more immersive and less like reading a visual novel if there's down time and mundane activities like going to school, working, shopping, or hanging out with other characters. Degrees of Lewdity (DoL) is probably the most successful example of this, but a lot of the big ero RPGs include life sim elements to some extent (TiTs, for instance).

Also, I just like seeing the worlds in lewd sandbox games. Since they're usually made by hobbyists/amateur devs, it's like viewing a raw slice of someone's mind, and there's often a strong sense of place that meshes weirdly with the lewdness. Like, DoL gives a pretty vivid impression of an English seaside town... overwhelmed by sex-crazed maniacs. Or take a more obscure game like ikkabod's Old School Blazin', with its 90s vibes and strangely unpleasant domination/submission mechanics. There's a disonance between place and content that I find surreal and darkly compelling in the case of DoL, and goofy but fascinating in the case of Old School Blazin'. Obviously, not all lewd sandboxes are that interesting, but the ones that are leave a powerful impression.

Anything with NPC schedules is also a plus. I like watching them going about their little routines.

At the same time, I also think not everything has to be a sandbox in order to have good gameplay. Some examples:

1. Lewd platformers and shooters tend not to have much in the way of sandbox-y mechanics, but some definitely have good gameplay. Others, like the old run-N-wape genre, are more of a grey area, in my opinion, where the porn is just kind of tacked on akwardly to the gameplay. Some even have a literal divide between the game and the pin-ups in an unlockable gallery.

2. I'm not sure whether certain types of lewd management games like slave trainers, card battlers, and strategy/tactics games qualify as sandboxes or not. Would you count them as sandbox games? They're often non-linear, and some grand strategy games (like eroTohoK, or XXXivilization) definitely have sandbox elements, but in other cases (like Portals of Phereon), most of the depth is in the combat/tactics, not sandbox/life sim stuff. And some of the old school slave trainers are more like VNs with preset plots underneath a thin veneer of management mini-games.

3. Adventure games (like Treasure of Nadia, or grubby old classics like the Frank's Adventure series) are another grey area between linear games and sandbox games. I like 'em, but they're not sandboxes in the conventional sense.

I'd also argue that not all sandboxes are created equal. There's a lot hiding under that label. Personally, I find stuff like DoL and TiTs much more interesting than any of Rockstar or Bethesda's sandboxes, where there's a lot to do but not much freedom of choice in how you go about it. Something like Red Dead is satisfying in its own way, but actually feels less immersive to me than lewd life sims with pixel art like DoL, where the stakes are higher and the world (on the surface) more mundane.

It's like the difference between picking from a long menu of numerous but very similar options, compared to a short menu of a few very different options. More choices does not always mean more meaningful choices.

TLDR: sandboxes are fun, and they're one of the main reasons I like playing amatuer lewd games (and especially, exploring the weird worlds that hobbyist devs create). But what consistutes a sandbox game is kind of hard to define, in my opinion.
 
A couple of games I can think of is summer time saga, or Hentai high school with a more in depth school management life sim.

An honorable or dishonorable mention, depending how you feel about sloth command, School game. (it's a school life sim.)
 
For your twinstick shooter needs. Certainly qualifies as gameplay. Decent voice acting, which is rare enough in itself.

Management. Freedom. It's a nice timewaster, bit lacking in the lewds maybe.

Tried the demo couple years back or so. Free roam, shooty shoot stuff. Was promising enough to keep an eye on.
