I saw mention of a "demosiac" file on another forum, I'm not sure if it was game specific or there's a general process.

It was most likely just for a single game, but with ai it may not be too far off

Those are often for games that use a "dynamic censor", meaning that they apply a filter/shader/effect over the genitals to censor it, and that can just be disabled.

For 2D games that is likely not going to be the case*, but if we are talking about 3D games, specially Unity games since they tend to be easy to mod in that regard (with tools like dnSpy), it usually can be done fairly easy.
Just keep in mind that the assets might not have proper textures and/or detailed models underneath the censoring, but at least imo creating textures for a couple parts is easier than editing hundreds of illustrations lol

*A notable exception being the Nejicomi Simulator games.

So, to put it simple, anything that is plain illustrations or pre-rendered animations requires manually editing all of them.

There used to be a thread about this on hongfire (which means 5ish or more years ago), from what I can recall there are a few games that use an image overlayed on the content, but most the actual content has the censorship built-in. I remember even if it was just an overlay which you could remove it was likely that you'd just find Barbie doll sexlessness underneath...
Tl;dr: the only 100% way is to get someone to draw over the censorship.

Very often the raw image files are censored. It's not an uncensored image censored at runtime, but just the censored image. Your only hope there is to see if the developer has offered an alternative version that is uncensored or see if fans have made their own uncensored version with alternate artwork to mod the game. You often cannot "de mosaic" the image because the mosaic reduces the image resolution and thus loses information. You can get much back from an 8x8 pixel image.

for future lurkers, some unity games only use a simple mosaic layer on top of their art which these bepinex mods can remove

as mentioned by the other posters, japan has been more strict lately with their censoring forcing the shipped art files to be hard censored so its less likely newer games will work with the mod.

I'd bet within the next 5 years there will be an AI service for it.

There are websites where the AI tries to decensor an image. But those are mostly for simple black/white bars.
Tested it once, result was terrible.

So the hope is that creators keep an original decensor file which they can publish.

In the Near future, AI replace the japan censorship bullshit. AI creates unbelievable great loli animations that we don't need their games. There are some forums like platinmods publish some game's patched version that uncensored version.

Yes, for the moment, if it's not an overlay, it might be necessary to recreate the images. However, I am also confident that AI image improving technologies will make decensoring easy in the future.

Nothing is worse than finding a niche game that looks fucking amazing only to discover it's fully censored without any way to patch it
