Those are simply not mutually exclusive, espescially considering that "loli / Milf" are fairly loose terms nowadays.
A loli could portray simply a petite young woman, or straight up a toddler, the latter of which i find personally disgusting.

And espescially in Artwork/ 3D media such as VN´s its very subjective sometimes in how both of these are portrayed.
