The reason many adult visual novels are so bad is that many writers seem to lack the ability to write basic English.
There are numerous spelling and grammatical errors. You might think I'm a grammar Nazi, but that's not the case; what I'm doing is highlighting how much most English-speaking schools have been dumbed down.
When most students leave school, they are nearly as illiterate as they were when they first entered school. This is how bad things have gotten in the last 10–15 years. I'm not blaming the students; I'm blaming the governments of English-speaking countries for allowing this to happen in the first place. Knowledge is power, and the fewer school leavers have basic knowledge, the better it is for governments to manipulate and use them when they become adults. It is difficult to believe, but there are many university students whose grasp of basic literacy eludes them completely. I have seen this phenomenon with my own eyes. I also have a number of friends who are deans of universities who state the same thing. And due to the inclusion law in many countries, universities cannot refuse students based on how poor their literacy is. As completely mad as that sounds, it's true.

It is very annoying to read because it is a mixture of very bad spelling and grammar and a nonsensical attempt at writing English. When I read a story, I don't want to spend my entire time trying to decipher what someone has written. It takes away any enjoyment and any immersion there might be in the story.
Many of the so-called English proof readers don't even speak English as a first language and are clearly not fluent. They don't even realize how bad their written English is. And since the developer of the game brought the English proof reader in because he or she is obviously not proficient in English, the role of the proof reader is to fix common spelling mistakes, grammatical mistakes, and nonsensical sentences. If the proof reader creates such a shambles, it stands to reason that the developer has no idea, as that was the reason they hired the proof reader in the first place.
Ultimately, the reason many adult visual novels are so bad is because developers don't bother to check if the proof reader is actually proficient in the language he or she is chosen for in the first place. Many adult visual novels fail within the first few sentences due to how bad the grammar is.
I also believe that this is also the reason why so many main characters are nothing more than children or just come across as mentally challenged. If the command of a specific language (English in this case) is poorly understood, then it stands to reason it is going to be written badly, and by this, what I actually mean, is that what the writer is trying to get across gets completely lost in translation, and no matter how good the grammar might be, it cannot make up for a very bad translation.
However, most of the time, this is not the case. The writer him/herself is just exceptionally poor at writing characters. When this is the case, the main character is going to be either a very unlikable child or a complete imbecile, both of which many adult visual novels suffer from and ultimately fail.
Note: When I say children or child. I am referring to the main character being a manchild. A very simple minded, cliche driven one dimensional unlikable character whose intelligence is likened to that of a young child, who doesn't know his or her place in the world and generally has no experience with pretty much anything, most if not all things go over the head of the main character, making him also seem like a complete imbecile too.
