There's one I know about but cant recall the name of it right off. I don't know about "grindy" but it certainly pushes you to learn some medical stuff. You literally listen to patients, check them out physically, and diagnose them. Depending on the content tags you like, it may not be your thing...- Gorion

Romance vs corruption
It depends on the game and story for me. I tend to naturally lean toward more loving/romantic options rather than evil options of any kind. At least for a first playthrough if I go for a second.
