I feel players are as much to blame as devs on this, I've seen games where the MC is like 9 inches and people will still go into the thread complaining that its too small.
F/D and B/S are my favorites, but U/N and A/N are also pretty fun dynamics. Weirdly I'm not into M/S, which might be the most popular one. I won't turn down a game because it has M/S in it, especially if its well written, but the scenario itself doesn't really do anything for me.
No, I'm never really self inserting here. I could never imagine naming the MCs after myself, for instance. When I'm annoyed at an MC it's because they're poorly written, not because I "can't relate."

I don't usually skip stuff, but sometimes the dev has attempts at humor that go on way too long and if I don't find them funny I'll skip those when they come up.
This has not happened. I keep a pretty strict boundary between fantasy and real life. As compelling as I might find a character I can't imagine "falling in love" with them.
Adult game? No idea, really. I've probably pulled off some marathons (relatively speaking) when I find a game that's new to me with a ton of content. I doubt any session has exceeded, say, five hours or so, though.

I think it generally makes sense for a character who is still experimenting but isn't quite ready for sex, but I agree that generally there aren't really a lot of scenarios where I'm like "hey, lets just go with a thigh job tonight."
I find it pretty easy to identify with the perpetrator rather than the protagonist in mangas, but it's a lot harder for me to not identify as the protagonist in games so I tend to stay away.
