    There's a difference between "big enough to show positions and perform certain acts" and "third leg". It takes a certain amount of length to perform acts like tit fucking, but when a regular joke in the media is "it's larger than my forearm" it can be a bit much.

    Probably the old flash games. Simgirl was probably the first, along with stuff like Frank's Adventure or the Meet'n'Fuck games. First download was probably The Sagara Family. Which I honestly think set the bar too high for VNs TBH, it was pretty decent erotica paired with excellent artwork.
