Use RPATool if you're concerned. The uncompressed size is almost exclusively from the PNG format images. There are well over 18,000 images and they are all pretty much between 1.5-5 MB a piece. It's about 49 GB just from uncompressed images.

Because most AVNs are done by one person or a very small group. Mostly in their off time and without the knowledge of their family. AVNs are a lot of work, and that makes it very easy for a title to get abandoned. Perhaps they burned out in general, or no longer have time to work on the game...
By the way, the update isn't a patch, really. It is a complete replacement for the script.rpa file in your game folder, so rename the downloaded "patch" and replace script.rpa. If you launch the game and the main menu screen says 0.65, you did it wrong.
